Automatically add checkboxes

If i want to add Next Row(eg: Gth row)  with the 5 checkboxes  as shown above by pressing button .   I mean person enter the name like (G,H,I,J.......) and press the button add. Please help me to write program for this task. this is really urgent frnds.
