to fetch data from ms word form into asp.net form

how to fetch textbox values of ms word document into the textbox of asp.net form........... 

i have attached a ms word form and want to fetch the input data of this textbox into the asp.net form.........how to do it??.plz reply soon ... its urgent..thanks
