1.Bug id 2.Test case id 3.Test case name 4.Priority and severity 5.Status 6.Email of devloper 7.Attachments if any 8.Assign to(Decided by Test lead) 9.Reported to 10.Completed date 11.Type of bug(Logicalcoding etc.) 12.Expected and actual results(depends upon company's requirements..)
