Crystal Report Generate Dynamic Boxes

Question

Hi,
I am creating one webpage in that i want to create crystal report , when user click on print then report will be generated in the form boxes like, 

in above record start from 1 to 6 and get retrieved from one single table and display, If any one have idea about this how to do it please let me know :)
