To the extent that it is sloppy for a programmer to modify a DLL so that the newer version breaks the users of the prior version DLL Hell is the result of sloppy programming. See also No End to DLL Hell! which explains ways in which the problem can still exist even with managed assemblies.

This is the situation which occurs when multiple applications install different versions of the same dll on the one computer. Sometimes all the applications will work fine with the latest version of the dll but more often some of them stop working altogether! This is a problem which has largely been solved by .NET's support for versioning.
