how to create a custom login form with FBA in sharepoint2013

Hi all I want to create a custom Login Form with rich branding with form based Authentication in sharepoint2016. can anyone suggest me the how to create this login form with FBA in sp2013 or 2016 stpe by step ? thanks in advance
