How to Display Workflow History Graphical Representation?

Hi
Can anybody give me the solution for my task .
i am using the application page when i submit form custom workflow is triggering i want to show the workflow history information in Graphical manner like below image i used SVG to create the circles and lines and rectangles but i can't able to bind the my list items to this Rectangle.give me the solution for this and i don't want to use Visio Services,SharePoint Designer fully i have to use C# only.
