first of all decide which form your going to use 1. webform or windows form. then in both cases as per your requirement you need following windows login questionaire report to save questionaire in database use following fields in single table Qno as integer Primary key Question Varchar() or text AnswerVarchar() or text UserID int RightAnswer Varchar() or text In design Either you can use panel or keep controls on forms
