If possible please try to understand what I have to add here... Please DO NOT confused between database storage and reporting or business requirements. These two are DISTINCTLY SEPARATE things. Your data should always be stored in a properly normalized way which means you should NEVER store strings like Record inserted by in database. Such strings are part of your view or select query and that's where they should be. For example: select Record  + case operation when 1 then inserted when 2 then updated when 3 then deleted end +  by  + userId from ... Hope you get the point.

As per your query you want to nsert the record in LOG_DESC as 'INSERTED RECORD BY- RITESH' For this you need to concartinate the your static string  INSERTED RECORD  with user name which can be retrived from the passed @userid. Declare @P_USER_ID int @P_LOGINUSER int @LogDesc varchar(200) set @LogDesc=' INSERTED RECORD BY- '+ (select username from yourUsersTable where P_USER_ID=@P_USER_ID) INSERT INTO RECORD_EVENT_LOG(USER_ID LOG_DESCCREATED_BY) VALUES(@P_USER_ID @LogDesc @P_LOGINUSER ); Check this query it may help you.
