it will easy if you get the all records and desgin them at page level and export the same rather than direct form database. if you are using mvc degin html page as you needed and show data and export same using 'Rotitiva' which you will get form nuget export you will get same excel/csv. and if you have only need to do using database. then create a temp table insert data as per columns insert blank row then insert data as your records and select the same.
