Problem in designing table

Question

Hi

        I am doing a project.. In that i need to design table.. I have to consider employee id and account type.. The problem is one employee can have any no of account type.. Now i need to enter same employee name repeatedly for which they belong to.. Is there any other way?
      
       I dont know how to design table for that..

       Can anybody help me?
