search in database table

Question

This is a form. i want to know that when i enter the consumer no and click on the search button, i take all the fields from database table of consumer and show it in these given fields. if i want to change any of the information in the given text boxes and click on the update button so it also updates the database table. please tell me how is it possible. give me the code if possible.
