1 . Create Master List with following Content Type Activity Area Category Document Economy Inquiry Document Economy Service etc. 2. ON your form (image : Content type) Add button upload file - and open your custom upload in model pop up (/customupload.aspx?CT=Document) pass your content type as Query param 3. Bind first drop down on load by querying to your master list and retrieve all ActivityArea 4. Based on selection of Activity Area - again query on master list and pull respective categories (http://www.c-sharpcorner.com/UploadFile/0e18a8/retrieve-items-from-a-list-in-sharepoint-2013-using-csom-jav/) 5.
