Web part costomization

hi i am quiet new to sharepoint & would like to know that can we create & costomize webparts without using sharepoint designer??

when i go to visual studio 2010 do everything sugessted by msdn then it says u have to install a sharepoint server.... what kind of server it wants...?? can u help
