When issue found on production. QA understands severity and priority of the issue. Understands how frequent the issue occurs and how much damage it can cause to the application functionalities data user experience and the performance of the application. Example - if login functionality is not working then it's the most severe issue and can block the entire user workflow and application becomes unusable for the user. Immediately report it via issue reporting tool he/she is using. It is very important to identify the steps to reproduce the issue to be able to understand the use case. Also extra information such as date and time of the issue occurence sample or production data which can help to identify incident while looking the logs or analytics. QA also tries to replicate the issue in dev and staging environment to understand if the issue is only in production or it is escaped from dev and staging testing. QA mentions all necessary details in the incident or ticket reported so that dev team can use it to understand cause of the issue and fix it.
