1. The costs of supporting multiple inheritance (dealing with the diamond problem virtual base classes etc) outweighed the benefits. 2. Experience of using multiple inheritance in C++ (particularly maintenance by developers who didn't write the original code) was not good. 3. No competitive pressure as Java didn't support it either.
